Vital records can be accessed from the website of the government of Georgia at http://www.georgia.gov/00/channel_title/0,2094,4802_5009,00.html. If you need records of births that took place in Georgia after 1919 , you need to fill up an online request form. The form can be accessed at http://health.state.ga.us/programs/vitalrecords/birthform.asp. Birth record can be requested by self, parents, spouse, grandparents, legal guardian and/or child. Fee for this record is $15 and an additional $5 would be charged for every extra copy.

For death certificates, you can fill up an online form that can be obtained from http://health.state.ga.us/programs/vitalrecords/deathform.asp or download the form from http://health.state.ga.us/pdfs/vitalrecords/death.pdf. In either case, you need to fill it up and send to Vital Records Department at 2600 Skyland Drive, NE, Atlanta, Georgia, 30319. Fee is same as that of birth record.

Vital Records can confirm the occurrence of divorce but for certified copy of the decrees can be issued by the county’s Superior Court Clerk. Download the request form from http://health.state.ga.us/pdfs/vitalrecords/marriagedivorce.pdf and send it to the clerk of the Superior Court where the divorce had been granted. Fee for this service $10 and payment has to be made in check or money order. Issue the check in favor of Vital Records and send it to 2600 Skyland Drive, NE, Atlanta, Georgia, 30319.

Vital Records hold marriage records from the year 1952 to 1996. If you are looking for records prior to 1952 or after 1996, place your request to the county Probate Court that ad issued the certificate. For the marriage records that took place between 1952 to 1996, download the form from http://health.state.ga.us/pdfs/vitalrecords/marriagedivorce.pdf and send to Vital Records at 2600 Skyland Drive, NE, Atlanta, Georgia, 30319 along with a check or money order of $10 and $5 for every additional copy.
